The Life Rooms empower people and communities to live the best way they can, offering an open door to a better day. Through health activation and community access, we support people to improve their health, wellbeing, and social circumstances. Our goal is to improve population health and inspire positive change across healthcare.

Our Social Model of Health

Community is at the heart of all that we do, and we aim to improve population health. Inclusion is the thread that wraps around our services and communities, from our fantastic accessibility projects to improve service user experience within our buildings, to the work we do every day with under-served individuals in our communities who may need support accessing NHS services and other key social institutions.

Health Activation supports people to become more engaged in their health, through developing knowledge, skills, and confidence that empowers them to take positive action to improve their health and wellbeing.

Community Access connects people to wrap-around support, understanding people’s needs and supporting them to take the first step to address those needs. This includes working with under-served communities and in partnership with community organisations to reach people where they need support.
