Who we are
Our team was established during the COVID-19 pandemic to identify people who were most in need of support and may not have actively engaged with services available to them.
We are a team of five - four Support Workers and a Team Manager - and we are based at The Life Rooms in Southport although we also cover Sefton and Liverpool.
What we do
We work collaboratively with other Mersey Care teams to identify service users most needing social support. Our team will contact them to check on welfare and explore any practical or social issues which may be affecting their mental wellbeing.
Our aim is to provide person-centred support in areas such as mental and physical wellbeing, housing, employment, benefits, volunteering, developing social interactions and learning. This is known as social prescribing at The Life Rooms and is offered through our Pathways Advice Service.
Although we are predominantly a telephoned based service, we are also able to offer a face-to-face service, providing essential support in welcoming new and existing service users into The Life Rooms and supporting their next steps.
